Wirkmechanismus
Typischer Wirkmechanismus der Beta-Lactam-Antibiotika:
Der β-Laktam-Ring der Penicilline bindet an bakterielle Transpeptidasen
⇒ irreversible Hemmung der bakteriellen Zellwandsynthese
⇒ osmotisch bedingtes Absterben proliferierender Bakterien
Bakterizide Wirkung.
Breitspektrumpenicillin:
Gegen viele gramnegativen Erreger wirksam, einschließlich Problemkeime (Pseudomonas, Enterobakterien).
Auch gegen einige Grampositive, jedoch sind Staphylokokken häufig resistent.

nicht säurestabil, nur i.v.-Gabe möglich
nicht β-laktamasefest,

Schwere, insbesondere nosokomiale Infektionen:
Pneumonie

Besonderheiten
Erweiterung des Wirkspektrums v.a. auf Lactamase-bildende Staphylokokken durch Kombination mit:
Tazobactam (ein β-Lactamaseinhibitor, BLI)
Aminoglykosid-Antibiotika
Kreuzresistenz mit anderen Penicillinen.
